Just leave that section blank and before the start of this para please do not forget to sign and date i.e. Declaration of applicant

Medicals are submitted for me. Any idea how long will it take to get passport submission request? Also is there any chance of cancelation of visa at this moment as well?

So my thought could be correct that another visa officer (I'm not sure what their position is called) would review ner cases at least to give further check and balance. Is this case or just 1 VO decides on ner apps?

I'm just thinking why very few per this week. Any idea?

There are couple of solutions.

1. Get your name changed in your passport. (The best solution but I am afraid it will take time)
2. Write your name in PERSONAL DETAILS Section of Generic Application Form for Canada as follows:

In 1 Write your family name as appear on your passport (misspelled one) and in 2 write your other family name you used (the correct one)

and in cover letter explain your situation to CIC.

As I understand principle applicant's travel history is mandatory and spouse is supplementary.
 
Dear 2171 Sep14 applicants,

As per SS there is a 18th August guy who got his DD cashed on 4th Dec 2014 !!!

There you go....it seems charging is not over yet for 2171 even though cap has been reached on 20th Nov...

It is a good sign, Don't you think???

But the problem is- latest date of application for which CC/DD charged remains 2nd Sep...

Hopefully, We will be hearing positive news very soon in sha Allah.
