Point table of your format is not mandatory...

No specific rules for filing your application, it's up to you, I didn't use any plastic files or anything like that
Again it is up to you to send details of the points you are claiming. personally, I didn't but I don't think it is a bad idea

make sure to punch all papers from top left corner and tag all together and put in a A4 bubble envelop, use clear tape to seal the envelop.
there is no requirement of CV and check list page will be fine no need of other pages..

I put everything into envelopes according to serial number mentioned in Document Checklist.

No, CV is not required at all.

There are no specific guidelines to pac your application....It is better if you use a water proof envelope.

CV is not asked my CIC and CIC themself will calculate the points even if you submit details.
 
Query regarding RPRF payment receipt - There is a client ID field there. Do we have to mention UCI ID? Also I guess the address will have to be the permanent address?

I don't think Passport expiry date should matter.

But which thing i hv to use to punch all papers? Suppose i use straplar to pin for evry single form. After that how can i punch all those forms? And another thing do i need to punch forms and docs all together?

DO NOT PUNCH OR STAPLE... DOCUMENTS SHOULD NT BE PIERCED.. I THINK ITS MENTIONED SOMEWHERE AS WELL..
I SENT THEM LOOSE BUT ALIGNED AS PER CHECKLIST AND CHECKLIST ON TOP...

Mention UCI ID in client ID field , provided latest address in address field (address where you want to get back your passport)
