Not only CAN you, you MUST do so. 12B requires you to list all employment in the past 10 years. You must fill it out completely and honestly. The instructions are very plain.

If she didn't fill out section 12B properly after you gave her all the information and she had your reference letters right in front of her, she has not done her duty and she has done you a great disservice.

Consultants are CRAP. But if you use one, at least examine your own application before it is sent!
 
is form 16 or ITR required to be included for a salaried person

Most recent statement will fork for the initial application. But your local VO later on may ask you for updated statements or several consecutive months worth of statements.
