No, it is not serious.

New applicants who is still yet to submit please note the below frequent errors:

Do last moment check: Most Frequent Errors

Ensure counting all the pages of form.
Ensure writing your full name at the last page of generic form.
Ensure selecting correct visa office
Ensure writing names in native languages
Ensure writing name, DoB, photo taken date at the back of photos
Ensure including self-addressed labels
Ensure while filling form IMM5406 by your spouse, s/he is applicant
Ensure Assets = Your property + settlement fund
Ensure providing original ECA, IELTS, and PCC
Ensure birth certificate contains your name, father's name, and your DoB
Ensure providing photocopy of degree/master certificates.

It is not fine. However, CIC will accepts this minor error as long as you give details/supporting of settlement fund. Relax.

I take it you paid by CC. The signature field is just for signing. And there is a box right above it for writing one's name as on CC (says name of card holder). I guess you didn't fill it. I am not too sure how important that is, but it is likely that might be checked as part of completion of form, I am NOT too sure though. You need to pray hard and regularly!! ???

I filled the box. My concern is, I put in the signature field only signature. But I guess it's fine

My understanding is that for charging CC manually, the agent need to enter Card Holders name as it is in the card, expiry date, CVV and amount in the system. If name, expiry date, and CVV match, payment is processed.

If you got UCI, send the documents with cover letter. They may accept it. You only loose courier charge.

Hey any one got Medical request recently from CPC-OTTAWA? I received PER on June 17th when can i expect my medical request from them ? I see in lot of peole before me got PER and none of them updated if they got medical or not .

In the signature field only one's signature is supposed to be filled. Don't need name along with it. So you're good then!!!! LOL unnecessarily worried!!

Yup, it is perfectly OK. Thought he did not put name of the card holders.

but I mentioned only 2 year addresses.

Those who got PER on or before 6th June, got MR. Don't get frustrated too much. Your MR is coming soon. If you got frustrated, many other also will get frustrated - who are waiting for AOR and forget about MR - Joking.

I would suggest sending the documents again with the cover letter mentioning your UCI, full name, passport number, and DoB. Keep the finger crossed.

Done added all my information , i was doing calculation on basis of previous Medical request, i was think i will be getting some where around next week but i see lot off guys from first week of june PER still waiting on Medical request. don't konw :'(
