That means adding brother and mother as dependents doesn't require additional funds right?

Bro, notarization doesn't mean that it must be done on a stamp paper. If a notary put his sign and seal also becomes notarized. If we prepare an affidavit from a notary, then it is required to be done in a non-judicial stamp paper.

If you want to say so, you need to know a country's law.

Your mother and brother should not be listed as dependents in any of the application forms even.
Remove them from any form in case you have added.
Dependents can only be your spouse and children.

I trust you are not confusing notarization with getting certified true copies, the process for which is outlined below (as per guide):

Certified true copies
To have a photocopy of a document certified, an authorized person must compare the original document to the photocopy and must print the following on the photocopy:

Who can certify copies?

In Canada:

•a commissioner authorized to administer oaths in the country in which the person is living

Family members may not certify copies of your documents.

Notarization, on the other hand, is done by a notary public on a stamp paper.

Thanks for correcting me in case I am wrong. Certainly I am no legal expert and will not claim to be one. I tried clarifying on a popular misconception between getting "certified true copies" made and "notarization", but if I made an error I am happy to be corrected. Cheers mate!!

Yes you need for UK as well. It took me 15-18 days after sending my application form and documents to get UK PCC. Their process is very simple as also can be found in the website referred above by a fellow user to you.
Let me know in case you have any questions on the UK PCC process.

I don't have a affidavit format per se. And I believe one should get a separate affidavit for each of the translated document. A notary should be able to help with the affidavit format, or ashif, Century and/ or other members here can guide you mate. Good luck! :)
