Did your parents signed on the Birth certificate?
If your parents signed then it is should be ok.
But if there is no sign at all, it is nothing but a piece of paper.
Contact CIC and send an affidavit as early as possible.
There are some people in our forum sent additional documents and CIC has accepted during the course of initial processing.
However Birth Certificate is very critical for application, I would recommend you to go ahead apply a second application.
on the other hand, send an amendment to CIC including all the details.

Friends those whoever sent amendments to CIC at the time of processing application, can you please suggest "how to approach CIC and sent additional documents, so that they will add to file?"

ECAS shows "Decision made, ......will contact you concerning this decision" BUT PPR not received yet. Is it normal with NDVO. No junk mail nothing...

Well the affidavit needed to be signed only by my only my parents’ names, place of birth, DOB is mentioned.

Parents Mr. xxx & Mrs xxx .However my parents couldn’t
register my birth at municipal Registry Hyderabad due to their oversight.
However my Passport and the SSC states the proof of my birth, since then there is a formal record or
registry of my birth exists.
I make the statements contained in this affidavit conscientiously, knowing the same to be true.

Congrats. Yes that's normal. You application is done with Security as well, so not they will now forward your application to Visa department. You will receive PPR soon.

Hospital Authorities say that they have up loaded the medicals on Nov 26-2014 and my documents reached NDVO on Dec 3-2014

My third line in ECAS is still not updated...

Does payment of Right to landing fees means that medicals are cleared and soon we may get a PPR????Any pointers

I called up the Hospital three times and each time they told me that they have already updated the medical results .....

RPRF DD getting clear has nothing to do with Medical, and calling Clinic wont help either.

They probably should have transferred your medical under "Review Required" or may be they didn't associate it to your account which is very unlikely.
Do you have any previous Medical history or something else which you think might affect your Medicals?

Nothing to worry about though.
