Agreed but I was not able to confirm on the CLB level .....they have categorically mentioned level 4 and the ielts grid has no reference of that

Please advise.
I have run into serious issue. I had travelled to Canada on a WP in 2013 for 3 months. My company started a new application for the same. I have just been informed that the NOC under which i last travelled was 2173 (LMO documents are not shared with us) and I have applied under 2171 this year. I have not mentioned the 2173 noc anywhere in my documents.

Please advise what should I do. I have already received my PER and the UCI number is the same allotted to me in 2013 travel.

Seniors please suggest next steps. I am now worried this might become a misrepresentation.

1) On what name should DD be taken for RPRF
2) Covering Letter: Can someone share a sample cover letter for the below scenario please
When applied for new passport, the following were the changes;
- Family Name
- Passport No.
- Address
3) Apart from Covering letter, is there any Affidavit needed? Also what should i include? Will the below suffice or should only particular documents to be sent?
- Old Passport bio pages
- New Passport All pages with notarization
4) What address should i send the documents to?

Please suggest when 3rd line will get update for me.

Med requested 11 nov.
Med done in hospital 30 nov.
medical report sent 4 dec by hospital.
same day I have sent my medical acknowledgement to ndvo which is also received.
 
Same here !!! Got 2nd line update 2 weeks back :(

I didn't mention nor said anything. Just copypasted the grid for you.
Don't read those notes - they are applicable to the primary applicant. Dependent should get CLB4 to claim adaptability points.

Friend, I think you are getting confused, you need to calculate points from each level.for individual modules. He is eligible.
