Can visitors apply for a study permit / student visa from within Canada?

If they are accepted in a college?

No, If they don't require a medical exam, they can apply either at a Port of Entry or a Canadian office in the US not to an office in Canada.

if she has a 6 months visitor visa, can she then ask for a student visa at port of entry?

you mean at the airport right?

and if that gets rejected we'll try the outside canadian visa office.

No, she can't use the airport, they are in Canada. It has to be a border port and she will need an acceptance letter, proof of fund to pay the International fees, and $10K for living expenses for the first year. Suggest you look at http://www.cic.gc.ca/english/study/index.asp
