what should be the format of cover letter for full application.

i want to inform CHC about some changed in my application forms, explain the reason for not providing the reference letter with duties of my current job and a few other points.

who it should be addressed to?
 
just address it to the visa office dealing with the country where you are residing. i don't think you should address it to anyone in particular since you don't know any of the officers.

Hi PankajK - yup replies so far accurate :)
we also had to notify of some changes in our original forms, we simply typed different letters addressed to the local CHC on each issue and put them into the packet just after the relevant forms in which we wanted to make changes.

Format by shibuya looks good. I would inser to suggest a Table (excel) in the letter identifying the form number, item number in the form, Original Value and changed value

Also Include the list of each and every document under the heads as explained in Visa form. If you are omitting any doc please explain the reason
