form IMM5406E Additional Family Information

regarding the form IMM5406E Additional Family Information, I am not sure what is right to fill out, do I put in the form the name of my husband (my sponsor), myself and the names of MY family ? Or also his family ? I am the applicant, my husband is the sponsor. Do we include 2 forms of IMM5406E ? where on one my husband fills out the infos and on the other one me ? Thank you in advance !

Sorry, I got it, I think ! I found a older post here, so just to make sure, me and my husband fill out each one of us a copy of that form, so we include 2 forms of IMM5406E, we have no kids. And one more question please, my husband got a brother, they don`t talk to each other since years and we just know he lives in Montreal, what should we put in the field for the address of that brother ? Is it enough to put `Montreal` ? There is no way we will find out the correct address of that brother.

If you look online, you can probably find an address for your brother-in-law. For example, 411.ca

Otherwise, explain the situation and everything that you did to try to find his address.

The sponsor DOES NOT need to fill out an IMM5406.

Yes, you can just put Montreal and include a separate sheet explaining that they have no contact and doesn't know his address.

only you need to fill this form not your husband who is sponsor

yes include only your family members details
