If you're working on the form on an Apple Mac actually need to use an older version of Adobe, the most recent version will not display the barcode.

Not Adobe Player, Adobe Acrobat Reader.
And I couldn't get it to work on a Mac anyway. I was sitting in Brazil with 12 people surrounding me yelling at me in Portuguese. I finally tried a PC and it worked fine.
