If your only dependent is a Canadian citizen, then the answer to question 3 is "1". (assuming you are the only one your wife is sponsoring)

You used the phrase "technically a Canadian citizen", which makes me believe that she doesn't have her proof of Canadian citizenship certificate yet. She will need to get that before travelling to Canada. The Canadian parent must apply for the daughter's proof of citizenship.

If you are applying outland, then your visa office will London. Don't write down the address of the visa office, just "London", they know where to send your application.
