For father and mother born in the United States do you just list City and United States of America, or do you list the State after the City? In our case it's a very common City name that's in multiple States.

It should be pretty obvious: if city name is not sufficient to identify the city, then it's not a complete address.

Or more simply: complete addresses in USA include the state.
