Form 5481 Sponsorship Evaluation

My wife is Japanese, here in Canada on an extended travel visa.
Our two boys are with us, both Canadian citizens with papers.
On form 5481 do I write my wife's name after question 6A and do I write the names of our boys in 6B, or do I leave this whole page blank?

If you did not sponsor the kids then it will be on 6B, but explain at the back.
Your wife name will be on 5A If you are sponsoring her, as I assume is the case.
