Hello! I want to send this form with my application because probably i will move out while my application is been processed and i want my husband to be able of change my address.
I'm confused with the part 3 of the form: if you've already submitted your application. In that part i should write the name of the office where my application was submitted, location of the office and type of application.
We are finish everything to send our application soon so we don't know what to do. Should i write the direction who appears in the package (we plan to use courier so it would be Robert Speck) or should i let that in blank? I'm scared, i don't want they return our application!
Also there is a space where the spouse should sign in case they want to release information to the same person. My designated individual is my hubby, so i assume it's not necessary his sign. Am i right?
