Fingerprint form field questions

Hi,

I went to UPS store for fingerprinting purpose.

He provided a fingerprint form.That form has blank ORI.

I have few doubts about filling the form.

1. Can ORI field be blank ?
2. what we need to enter in the employer field? our employer name or fingerprint agency name ?
3. Do we need to fill OCA and FBI fields?
4. Do we need to send separate fee while applying for husband and wife..

No it doesn't, the FBI doesn't care.

From your link

You should:

* get a cover letter and fingerprint card from the FBI. You can download these from the FBI website in the Related Links section at the bottom of the page or write to the address below and have them mailed to you.
* have fingerprints taken. You can have this done by:
o a local police department or
o a private fingerprinting business.

Fingerprint Card

Obtain a set of your fingerprints (original card, no copies). Your name and date of birth must be provided on the fingerprint card.
Fingerprints should be placed on a standard fingerprint form (FD-258) commonly used for applicant or law enforcement purposes.
Include rolled impressions of all ten fingerprints and impressions of all ten fingerprints taken simultaneously (these are sometimes referred to as plain or flat impressions.)
If possible have your fingerprints taken by a fingerprinting technician (this service may be available at a Law Enforcement Agency.)
Previously processed fingerprint cards will not be accepted.

I got my finger printing done today from a technician and don't know what to write as ORI number?

I read on the forms ORI is a required field.

Friends,

I am from USA I am also having similar issue i bought the finger print card from amazon since the law enforcement office does not have it.They are ok with the card but the problem is they dont know the ORI and OCA number specific for canadian immigration.

Guys please help me on this i stuck in this ORI and OCA information.

How did you move forward with ORI field?
I read here(https://www.canadavisa.com/canada-i...gency-number-for-canadian-immigration.150131/) on the forum saying it is not needed for immigration yet, I want to confirm this.
Can I leave it blank?
