Filling the PR application under QSW

Hello all,

Q1: I am trying to fill out the PR application under QSW, and it asks for the CSQ number in the Generic Application Form. I am wondering what that might be?

Q2: For the National Identification Document, the one I have will expire soon. Does that matter?

Q3: In "your travels" form (IMM 5562), is it right what I understand, which is I need to list all my trips to countries other than my country of origin or the country of my current residence?
