filling out study permit form

Hello,

I am working on my application to extend my study permit.

Can anyone provide a sample on the proper way to fill out these study permit forms?

I am confused with the proper way of filling out blank spaces. So where should I put 'NA' or 'Not Applicable'?

When I finish filling out the form online and then click the 'Validate' button at the top, then some of the blank spaces gray out, and then I can no longer input any information ( not unless I print out the form and use a pen on the grayed out space ). Also some of the empty spaces ( e.g. for fax number ) do not get validated, so does that mean that I can leave these spaces blank without putting anything on it? ( not even 'NA' or 'Not Applicable')?
