Filling "Additional Family Info" page

Hi all,

Guys I have a problem with filling "additional family info" page for my wife. it's all clear when i fill in for myself, where i write my parents' name, sisters, etc. but with her page I find it diffuclt what to write in the in the first section. Should we write her name by side where an "Applicant" is written?? In fact she is not applicant, that's me. But on the other hand if I write my name, her parents and sisters will appear as mine.

How did u solve this problem?

She completes the 5406 as the applicant. She is an applicant.

same question I have, should my wife after filling her name as the applicant should she put my name as the spouse or partner and does she has to sign this section (sectionA) as well as B and C.?
