I am thoroughly confused.

Do I need to fill up a certain form and send it with my Demand Draft's for RPRF at CHC-ND?

CHC-ND has sent a letter along with Meds and askd me to submit PPs and RPRF.

But the entire package does not contain any specific "Permanent Residence Fee Payment Form".

Again, the letter says "Please send the following info in a single package within 45 days
from the receipt of this letter and return "this form" with the requested docs"?

What form is it talking about? Is it they are referring to the letter as the form?

Pls suggest asap. I am ready with everything, but could not find a RPRF Payment Form anywhere on the CHC website.

The website only contains IMM 5612, which is a Visa Fee Payment Form.

My wife is principal applicant for CIC applicaton, as i am also sailing in same boat of pending state. But we have new baby girl in our family.
a) So what is procedure to inform cic for new baby girl ?
b) which application forms to attach to be send ?

No form exists for sending RPRF. Just add a cover letter to the DD, indicating your particulars & DD details. The Form they are referring to is the request letter itself (for easy logging & correlation when received by them).
Also do send them the original fee/receipts given for the medicals by your DMP & any old Passports of all family members.

Things seems to have started moving for MANILA Pre June 2010 applicants. Few Medicals requests have been reported lately.
I hope CHC New Delhi follows the same. It is important to remind us all that Manila and New Delhi are one of the Fastest in the World.
 
Meds done on 10th Sep. Meds sent by doc office on 13th Sep. E-CAS nt updated as yet. Don't they
write sumthin like "Meds rcvd by CIC" on Ecas? Pls suggest.

yes usually they do but, ECAS is not 100% reliable so, dont worry... Just enjoy the moment and have faith in almighty... you'll be fine.
