Fees paid information

On the fees paid, I had to use a family member's credit card since I do not own one, but I gave them the money to pay for it. I filled out the receipt with my DOB, phone number and address, and wrote a letter explaining who's credit card I used and that everything other than the Name on the paper and the credit card information was mine. Was this the incorrect way to do it? It's the only copy I have, and I don't know how to print off another receipt. Should I include my family member's DOB and phone number/address on a separate sheet?

When you're done paying, the website ask you for an email address and a copy is send to this address so you can print a copy.
