can anyone please confirm if we have to send fee payment form to CIO in case of back draft as this form requires details for credit card only and asks for nothing else.
do we need to fill it in case of bank draft and fill columns with NA and attach bank draft with it.
please help
 
This form is must irrespective of the mode of payment.

You need to enter details of the amount in the table and total amount and for point number four check the second check box.

Write N/A under CC details. Do not forget too put in the date as this is one of the common mistakes that we see on the forum
 
i think you meant to say option 4 . first check box for bank draft...
this form gives options for Name and Date within credit card table.
so where do we need to mention date,Name and do we need to sign it as i read somewhere that we need to sign all the application forms.
and one more thing i have to attach bank draft (copy or original?) with this form.
 
Follow Step 1 to 4 like calculate the fees, and check the ckeck box for draft
