Oddly enough I find them a lot more straight forward.

My application was returned today. I had not included IMM5406. Could someone please confirm to me how to fill this form? I read on this forum that we need to print it out twice one completed by me and the other my spouse. Is that correct?

same thing happened to me... I was told to submit this form. WHAT A MISLEADING NAME THE FORM HAS.
I'm being sponsored by my Canadian Husband.
From what I understand, the form is to be filled up by the principal applicant. In my case it was myself. I'm only applying for myself so I only filled up information of myself.

I haven't received AoR yet maybe seniors could explain better.

We are talking about additional family form? Yes, the person being sponsored has to fill out this from!

If you are the only one included in the application - then only you need to fill this form out (Principal Applicant). Your spouse (sponsor), does not have to fill this out. I hope this explains it well enough.

I'm pretty sure he needs to mail it in. I don't think they would allow him access to the mailroom in person.

Do you mean E.Eqra?
I sent mine a little over a week ago and it was signed by E.EQRA in the Mailroom
Must be the name of the person! Nothing to worry about
