Did they print the certificate on white paper, or are they still using the old blue security paper?

Hi, Can anyone please tell me how can I have my fingerprints taken ? Can I print the form and take fingerprints myself or I should go to an agency ?

there is a specific fingerprint form FD-258. you can get it from fedex or staples
 
You can print fingerprint form to a regular white letter size paper. It is allowed by FBI and they accept it.
 
this does not preclude further criminal history at the state or local level

Yes. It's a common template for everybody without any history.
