I sent my fingerprint card off to the FBI last week, but after I got home from the post office, I discovered a form that I failed to include. It was a sheet with my info on it, says I-783 at the top. I included the fingerprint card and the form with my credit card info for payment. Will I be ok? I know it takes 8-12 weeks to get it back and I don't want to wait and find out I have to send it in all over again. My partner and I will be common-law around New Years and I wanted to send the application in by the end of January if I could. All the info is on the fingerprint card and my payment form.. will this do?

Isn't that the Applicant Information Form?

Since that form has all of your personal data (including your physical description), and requires your signature, it's likely to be an issue.

They once had their phone number posted on the website, but since the delay in processing was announced, it's no longer there.

Oh no. That is the application form. Your best bet is to start from square 1 and redo your fingerprints and send it in again (with the application form).
 
oh no :( I signed the card too though as well as put all the info there. Guess that's not enough. :( I have an extra card (they did two just in case). So it won't mess it up if they get two?

Extra set of prints? That's good.

I doubt sending in another application will mess anything up. Most likely they will simply mail back the first set with a note that you are missing the form anyway.

I have been calling them periodically ever since they announced the delay, so there's that.
 
Thank you so much for that phone number! The customer service rep told me just to fax the missing form to them with a note explaining what happened. I hope that works! *fingers crossed*
