Is my American wife legally allowed to use my surname since getting married - as her own - on the application or does she have to use her last name? Does she need to apply for a name change?

She can't use your surname legally until she legally changes it. Until then, she has to use her own surname on all official documents. Getting married doesn't automatically allow her to use your surname legally.
