Eye colour and height

Hello everyone,
I got request for medical exam from CPC Ottawa and request to provide the eye colour and height for me and my accompanying family members.

In order to continue processing your application, the following is required:

Medical examination for all (Medical Instructions will be sent separately via e-mail within the next 48 hours). “To update your physical description, please provide the eye colour and height for yourself and all of your accompanying family members”.

In what form should I provide them eye colour and height?

Should I put that data into table and submit to case processing center to the following address:

I thought Eye color and height information was part of IMM 008 generic. Isn't it?

Yes, we have written our eye colour and height in Generic IMM0008. For some reason they want us to confirm (update) that data.
