extension of visitor status

We applying for extension of visitor status for the second time. First time we were granted extension for 6 months and were required to undergo medical examination.

This time in the list of required documents Im asked if the medical examination has been completed in the last 12 months. Yes it was completed for our last extension and results were sent to Visa centre by the medical examination centre. I dont have any documents left to show for it. So question is- How can i show a proof for medical examination now that im applying for second extension?

Or should i say that medical examination has not been completed ? that way the proof is not showing up on the list of required documents.

Where is your receipt? you misplaced it?

You can call the doctor's office and ask for a copy, some hold copies for receipts, some don't.

If you can't prove it then you will have to mark as "no" and take them again.

Depends. If nothing has changed then yes, but for example for a kid or if you went from long hair to really short, then you will have to use a new one.

That is such a good question. I had a new pic taken for my PR application because my extension pic was a year old. I wasn't sure if CIC had requirements about the date & stamp on the back that was stamped by Shoppers Drug. Now for my next extension I will use the one from my PR application (Sept 2015) there will only be few months apart. Plus I like the latest pic better.. LOL haha
