Extension for Medical for Super Visa

My mom received a letter to go Medical exam for Super visa. But recently she was not feeling well and the doctor detected pneumonia.
So, what's the most appropriate & convincing reason and the period I should request for to get an extension?

She is unavailable to go for the exam as she is out of her home town.
What about the maximum extension period?

The medical can be done anywhere, so not being in her hometown is not a reason. There is no max extension period.
