Extending work permit

Looking for some guidance on the correct form to use so we can extend my husband's work permit.

We filed out inland spousal sponsorship application 3 years and 6 months ago. Apparently the hold up is due to a back log in security background checks. Its been a ridiculously long journey. We had to do a 2nd set of medicals this past March because the first set has expired. My husband received his work permit in Feb 2014 that was valid for a year, we extended it and they had issued a 2nd permit that was valid through till Feb 2017. We want to submit to extend the permit again, but I'm unsure what form to use. We had a lawyer that did it for us the last two times, but we've since stopped using his services so I'm just a little lost by CIC website and want to ensure we use the correct form.

You can apply online through myCIC or in paper: http://www.cic.gc.ca/english/information/applications/extend-worker.asp . When filling out forms for OWP, you leave the employer information empty, and supply a copy of your marriage certificate.

EDIT: Also include your UCI if you are filing by paper. They can locate the PR application that way.
