extending multiple entry visa, out of canada. possible?

My Multiple-entry Visa is gonna expire next month.
I intend to visit Canada for the next few months to come.
Can I extend the visa? I am in the States now.

You apply for a new one, at one the Canadian offices in the US.

My mom is going back to Canaqda on July 2012, and her multiple visa will expire on September 2012. Is it ok no to pay/renew her re-entry visa?
