I applied for study permit together with visa in October. However, I got approved and received my study permit but not my visa. I want to apply to extend my visa again but there is a problem that my CGPA currently is suffering due to my sickness. I am afraid that I would get rejected. What would happen if I have a valid study permit but my visa got rejected ?

Visa (TRV) is only for traveling to Canada, so you would only need it if you leave and want to come back. GPA should not effect visa application, I don't even think its a requirement to show transcripts for visa, talk to immigration at your school.
