Extend my PGWP

Hello,
I came to canada with a study permit for 3 years and my husband with a work permit . I got my PGWP on Sept 2018 and is expering on March 2020, because my passport expire on that date. I have a full time Noc 0 job and all my payslips. So, I have some questions.:

1. How do I apply for an extension, because online there is different kind of work permit, which one should I apply for extention for?

2. Can I apply to extend both work permit together in one profile, or have to be separate?

1. There is only one work permit extension app, with IMM5710.

2. You can apply together with your skilled job proof and payslips.
