Express Entry- Personal details (Immigration history and citizenships)

I declined my ITA earlier and I am re-applying through the same profile. What should I answer to "Have you applied to Immigration, Refugees and Citizenship Canada before?"
Should I answer Yes/no? If my answer is yes, then the file number of my express entry would be the same as before. Is that acceptable?
Please help.

Have you applied for study permit, or work permit before? If you have done so, then your answer should be "Yes" to the question.
Your UCI number for PR will be same as your other applications you have filed in the past.

No, I have not applied for any study or work permit before. I had applied for Express Entry and I got my ITA in Nov '20; but because of document issues, I had to decline my Invitation. So I'm still in the pool, but I do not know the answer to "Have you applied to Immigration, Refugees and Citizenship Canada before?"
