Everyone help, how to write a good explanation letter to the visa office

I had been refused a visit visa in the past, which I forgot include this information in the documents. Now I want to explain to the visa office, but I do not know how to write a good explanation letter, plus English is not my native language. So I hope someone can give me some advices, or it is better to have a sample .

Write a letter in your own language, then get it translated/certified into English from some notary public /barrister/translator, etc in your city.
Send that to the visa office.
