Noc 6322 cook(skilled job), Employer must provides air ticket and accommodation, and health insurance?

No.

Also, health insurance is provided by the province and not the employer.

A site I just found says that depending on the high or low salary, the employer must pay for transportation, accommodation, and insurance. Are there any exceptions?

For low wage roles the employer must provide transportation. But they are not obligation to provide accommodations. They need to make sure employees have access to suitable / affordable housing. But this doesn't mean that the employer pays for those accommodations. Note that cooks are not always low wage.
