I am filling in my employment history for the citizenship application.

One of the companies I worked for has completely relocated to a different address (they were in the process of moving while I was in my last month). On the citizenship application, shall I put the new address or old address (the one I actually worked at)?
 
I have the same question, does any one know, if company's address changes within the last 5 years, which address to enter new or old or both?

You need to put the address where you went to work. So yes, if the company address change meant you also changed where you went to work (and it's not some headquarter move that didn't affect you), list your job twice with the relevant address and date.

I worked at both the locations (thought it was 2 days in the office).
So Do I have to create separate entries for the address change - Something like for example:

2021-2022: Company-A & Designation-A & Address -A
2022-2024: Company-A & Designation-B & Address -A
2024-Present: Company-A & Designation-B &
Address-B
