I want to ask about some fields in the Generic Application form for Canada. The first one is about other name(s) used by the applicant. If the applicant changed name (e.g. from Jane to Anne), do I put the other name inside parentheses when I enter this name in the name field for no. two question in personal details part? And also why can't I enter details in other fields like the date of last entry to Canada?
