I have more than 5 years of work experience in Pharmaceutical industry.

I hold a Masters degree in Pharmacy, do i need to get Transcripts from University for my Bachelor's degree as well?

That depends on the organization doing the assessment. If they require your undergraduate transcripts, you'll have to send them. Check with the organization.
