Driving License (New Jersey - Ontario)

Hello,
I stumbled on many threads but still it's not clear to me what all I need in terms of documents to get my NJ license exchanged in Onratio.
- My license is valid and have few more months on it.
- I figured that I need to get a driving history abstract from NJ DMV.

Question:
- What other documents do I need?
