Yes, it is the only form which needs physical signature.
Fill it and take a print out. Sign it, scan and then upload.
Telling you from my personal experience. I also faced the same problem.

I applied on 16th May, online and got approval on 17th May. Submitted passport on 22nd May.....sent by VFS Chandigarh to Delhi. Got stamped passport on 29th May, 2018.
