Documents to add Wife to CEC PR application

Hi,

I applied for PR in CEC category when I was single. I got married recently and would like to add my wife to it while my PR application is in process. I email CIC about this and they replied asking for a bunch of document including these 2:
- Principal Applicant’s Relationship Assessment Questionnaire – completed by the principal
- Spouse’s Relationship Assessment Questionnaire – completed by spouse or common-law partner

What kind of document are these? Where can I find this questionnaire?
