Documents-Spouse name change after marriage

Hi Friends,

I want to know that if my spouse's middle name is changed after our marriage what kind of documents are required to show change of name?

I have marriage certificate.

is Marriage certificate with LOE is Suffice?? Or any additional documents required??

You will need these:

- Marriage certificate
- Legal name change certificate (Name change affidavit) in front of a notary. Just contact any notary, he will have a format for this.

Both of the above are sufficient & also mandatory for your purpose.
