Documents required for submitting application for OINP interest letter in 45 day

Dear members,

I received letter from Ontario stating Ontario is interested in me on this 4th Dec. I wanted to look into the further procedure I need to do and documents requirement.

I am trying since Monday, but CIC login site is not working. It says temporary out of service for maintenance.

Can anybody please share the further process and document requirement for the OINP.

Your help is highly appreciated.
