Documents required for Skill assesment

Could you please tell me what are the documents I should prepare to do background verification/skill assesment?

I want to apply for PR under skill workers program in this year. I am a s/w engineer with more than 8 years experience.
What exactly I should do now? I am already preparing for IELTS.
