Hello everyone,

I just got my appointment for my medical and I was told to bring my passport, ID and my application documents. When I asked what this entailed, the lady said that I should have received a form with an IMEI number (and possibly a list of panel doctors) on it. I figured this would be my invitation letter but I still want to make sure. I haven't seen anything about any medical document I'm supposed to bring on my checklist, is there anything I'm missing?

Where are you applying from?
For my medical appointment, I was asked to bring one passport photo and ID only.
The CIC website has a list of panel of doctors that you can choose from for your medical exams, and ONLY those listed is recognized by CIC.

After my medical exam I received an E-medical information sheet that has my UMI code (the code used to track my medical records).
The nurse told me this is the information that I need to upload as proof for my medical exam.
I had my medical exam in Canada though, so I am not sure if it's all the same in other countries.
Though I did call and ask one doctor, the receptionist was rude and said there's no such thing as applying for permanent residence and said she never heard of Express entry. Also asked me to bring in a medical form (I had no clue what she meant and did not bother continue asking since she simply told me to call CIC and ask them).

If it's an upfront medical, you won't have a form yet!

My doctor had e-medical system, so I did not have to bring any photos.

I also had to bring cash, though. :p

I see. The lady said they need the said form so that they can connect my application to my examination. If that doesn't work she said they could try using my passport number. My invitation letter already has a barcode on it so I'll bring that with me just in case. I'll call them again today to confirm.

They must issue you a UMI number, because you don't have one yet (if they use e-medical) It's then CIC's job to connect your application with your medical information. This is why you need to upload the e-medical information sheet.

I'm unsure what the process is if your panel doctor doesn't have e-medical. I would highly recommend choosing a panel physician who has it if there is a choice.
Thank you for the clarification. I just called them back and when I told them it was an upfront medical, they said that I only need to bring my passport with me. So all seems good. :)

I am curious, which category have the doctor chosen for you? Non ede worker? or maybe ede family. I am confused. I am having my medical tomorrow at 9;30 am and still not sure which category specify is the best. :eek:

Where are you from? Most likely your doc with choose non-ede worker. Ask him to additionally write in the comments section that the exams were performed for permanent residency purposes.

Did you ask the panel physicians that are listed on the CIC page? http://www.cic.gc.ca/pp-md/pp-list.aspx

After a lot of emailing, phone calls and involving a third person, I managed to get this resolved. Going for my medical on Thursday! ;D
