documents for IMM5669 Background/Declaration

hi... just want to clarify..is there any document i need to provide in IMM 5669? The checklist didnt say to provide proof or copies of employment certificates or school diplomas..pls help, thanks ; )

I believe you wanna submit it for PNP. You don't need to submit any documents with this!

thanks, mahfuzul80.... have u tried it before? i believe u also applied for mpnp online right? then submitted ur application for permanent resident visa via mail... and u did not include any document for imm5669(background declaration)...sorry to ask..i just want to confirm if im not gonna have any problem if i dont attach or include any document to support my imm5669..ur reply would be a very great help for me, thanks...

Hi, I did not send any of those documents too. I just followed what it says on the checklist.

Make sure to double check the documents that's on the checklist. I just got my UCI number just today. Keep praying and think positive.

You don't have to attach any documents if they're not required on 5669. Just prepare your application package according to the Checklist form.

Thank you. Some people are saying need to attach paystub and reference letter or certificate of Employment to prove the personal information on 5669. If that’s the case, I won’t because it’s not indicated anyway to include them.

All required documents will be listed on the checklist form. For imm5669, officer just wants to know your education history, personal history and etc...

Thanks again. That means no proof to include on what I put in. I can include my proof it’s just that on the package form it says not to include documents not being asked.
