Hi All,

My parents recently received an email from CIC with following verbatim;

In order to assess this application, updated copies of the items listed below are requested. Please note that if we do not receive the items listed below within 60 days of the date on this letter, this application may be refused.
• Application form (IMM008) for yourself
• A Schedule A form for you and one for all accompanying dependants over the age of 18.
• An Additional Family Information form (IMM5406) for you and for all accompanying dependants over the age of 18.
• The documents listed in the below Document Checklist for Family Class – Parents & Grandparents
• We also require that you and the other family members who are included in this application undergo complete medical examinations at a Designated Medical Practitioner. You must take this letter with you to the Panel Physician (formerly Designated Medical Practitioner). Please see Instructions for Medical Examination below for additional requirements. The issuance of medical instructions does not constitute final acceptance of your application.


I am kind of confused on what does this mean? Basically, they have asked to send all the forms/documents again along with passports and medical results. The language is quite ambiguous. From other members' experience, what is the success rate of getting the residency after this?

Also I have one important request; I couldn't find one form that this checklist is talking about.

- A travel history form for you and for all accompanying dependants over the age of 18, declaring all travel undertaken.

Can anyone point me where I can find this form or how it is supposed to be done? I did search the CIC website but couldn't find any. Thanks.

Since there is such a wait in this category I am assuming it has been a while since you filed... Seems to me that they are starting to processing the file now and want to make sure it is up-to-date... So just refill the forms and do the medicals and send them in. As far as the travel documents go, I came across a few seems like it depends on your Visa Office... But they all look alike and ask the same questions so it prolly won't matter which you use here's a link to one below... Good Luck!

YES, CIC Delhi ask for same docs again ( don't ask why ). Once you submit passports and take meds, takes about 2 months
