If a doctor immigrates to Canada and is not licensed to work as a doctor there, they can do any other job they are qualified for. Some have found work in medical administration and some have taken the easier licensing exam for nurses and choose to work as a nurse. If they want to change fields completely, open a business, whatever, that is allowed too.
