Do we need USA visa in advance to travel by air

first time you apply for the visitor visa its based on the immigration officers discretion, I got 5 years and my friend got 1 year, it does state if you abide by the conditions given when you were issued your firsr visitor visa , when you go to renew it they will give you 10 year visitor visa.
